چکیده

Rationale & Objective Suboptimal dialysis preparation of patients with chronic kidney disease (CKD) is common, but little known about its relationship to psychosocial factors. This study aimed assess patients’ attitudes access creation and identify factors associated intentions regarding outcomes. Study Design Prospective cohort study. Setting Participants 190 stage 4/5 CKD not receiving treated at 2 hospitals in Singapore 128 their family members. Predictors Self-reported measures illness perception, health-related quality life, toward creation. Sociodemographic clinical were also obtained. Outcome Intention create an arteriovenous fistula (AVF; ie, proceed vs wait see) time a functional AVF. Analytical Approach Exploratory factor analysis (EFA) was undertaken construct internally consistent subscales for newly developed questionnaire Logistic regression cause-specific hazards models conducted outcomes, respectively. Results EFA (explained 50.1% variance) revealed 4 domains: concerns, need dialysis, worry cost, value access. A high risk intention delay (51.1%) found among despite early referral education. Multivariable (R2 = 0.45) showed that the greater perceived from (odds ratio, 2.61; 95% CI, 1.46-4.65; P < 0.001). Limitations Limited generalization, as only those already nephrology care studied. Conclusions Approximately half studied planned The evaluate may help individuals whom decision-support programs would be useful. These findings highlight understand address concerns

Aneurysmal Dilatation of Dialysis Arteriovenous Access

Arteriovenous fistula is considered the “ideal vascular access” in patients with end stage renal disease (ESRD); however, its creation is not without complications. Aneurysmal dilatation is a poorly defined complication which, can potentially lead to access loss and life threatening hemorrhage. Increased venous pressure due to proximal venous stenosis along with repeated cannulations at the sam...

Arteriovenous Access

Complications of vascular access lead to morbidity and may reduce quality of life. In this module, we review both infectious and noninfectious arteriovenous access complications including neuropathy, aneurysm, and high-output access. For the challenging patients who have developed many complications and are now nearing their last vascular access, we highlight some potentially novel approaches. ...

Obesity and access to kidney transplantation in patients starting dialysis: A prospective cohort study

BACKGROUND Obesity has been linked to poor access to medical care. Although scientific evidence suggest that kidney transplantation improves survival and quality of life in obese patients with end-stage renal disease (ESRD), few data exist on the impact of obesity on access to kidney transplantation in this population. OBJECTIVES We aimed to characterize the relationships between body mass in...

Successful restoration of arteriovenous dialysis access patency after late intervention

BACKGROUND Arteriovenous dialysis access may be lost due to stenosis and thrombosis. Patency may be restored by thrombectomy or thrombolysis, but this is often not undertaken when the presentation is delayed. The success rate of delayed intervention is largely unknown. METHODS In this single-centre study, we identified all instances of arteriovenous vascular access (VA) failure treated with a...
